Add "Scope" text to "Scope" options

In the documentation, it is stated the creation form requires "A scope", but "Scope" is nowhere to be found on the form.

I recommend adding a "Scope" label just before the checkboxes for "Applies To Session Hosts", "Applies To Devices", and "Applies To Servers".

I would then update the documentation to state "Scope" instead of "A scope" and also show the new Scope selection area in the screenshot.
